Early in the spring semester, you will receive an email from the Graduation Office requesting that you to complete the “Graduation Checkout” in e-Services by Friday January 26, 2007.
You will print the Graduation Checkout documents (see below for specific information for undergraduate and graduate programs), sign and submit them to the Office of Enrollment & Student Services (Room 1009) in Theater Row OR mail them VCU School of Nursing, ATTN: OESS, PO Box 980567, Richmond, VA 23298. These forms must be submitted to the School of Nursing’s Office of Enrollment & Student Services by February 1, 2007.
Traditional and Accelerated Bachelor’s

You will receive return envelopes and scantron sheets from OESS to use with the above. Be sure to return your materials to us in the envelopes we will provide. OESS will not accept your evaluations and surveys if they do not come in the envelopes provided.

Master’s:

- Graduate End of Program Survey (you will receive this in the mail from OESS)
- Graduate Preceptor Evaluation (you will give this evaluation to your preceptor and they will either return it to you or mail it directly to us – this is only is you are taking a practicum class). Your preceptor is evaluating our program, not you so this will not be part of your official record (nor will your end of program evaluation).
- Survey of Employment Patterns survey

Application for Licensure in Virginia
Information Instructions for Filing Application for Licensure by Examination for Registered Nurses can be found on the following website - www.dhp.state.va.us/forms/nursing/RN/RNexam_application-and-instruct.doc
Note - Application for licensure must be submitted at least 60 days prior to the first day of the month in which you expect to take the examination.
Your application can be notarized by Angela Moseley (Room 3083 -- Theater Row).

NCLEX-RN Examination Registration

You must submit a Transcript Request form to have the Registrar send an official transcript to the Virginia Board of Nursing (or another board of nursing) immediately following completion of the program. The official transcript must have that the BS degree has been awarded. It takes approximately 2 weeks following graduation for the degree to be posted on your transcript.
Cap and Gown
The Grad Finale will be held at the Larrick Student Center (641 N. 8th Street) on March 19, noon - 6:00. Students who cannot attend can order graduation regalia online at http://www.jostens.com/ starting on March 20.
School of Nursing Pin (optional)
The MCV Campus Bookstore has a wide selection of nursing pins available. You can order your pin at any time at the bookstore.
